Dr. Hazen is one of the founders of Andersen Eye Associates. He specializes in topical, no-stitch cataract surgery and has performed more than 10,000 cataract surgeries. Prior to founding Andersen Eye Associates, he was in solo practice for 20 years.

Dr. Hazen received his medical degree from Kirksville College of Osteopathic Medicine and completed an Ophthalmology Residency at Saginaw Osteopathic Hospital and Metropolitan General Hospital in Pinellas Park, Fla.

He is a certified member of the American Academy of Ophthalmology, OCOO, AOA, , the Saginaw County Medical Society and Saginaw Osteopathic Society. He is an assistant clinical professor at Michigan State University and Ferris State University.

Dr. Hazen is a member of the active medical staff at Covenant HealthCare, St. Mary's Medical Center and Health Source Hospital in Saginaw.
