Dr. Hazen joined Andersen Eye Associates in 1999. He received his medical degree from Wayne State University and completed his Ophthalmology Residency at Kresge Eye Institute/Wayne State University. Dr. Hazen was accepted into the Oculoplastic Surgery fellowship program at Kresge Eye Institute/Wayne State University, where he completed a fellowship in Ocuplastic Surgery. Since joining Andersen Eye Associates, Dr. Hazen has completed approximately 6000 cataract procedures.
Dr. Hazen is active in the American Academy of Ophthalmology, the American Society of Cataract and Refractive Surgeons, the Saginaw County Medical Society and Michigan State Medical Society. He is an assistant clinical professor at Michigan State University.
Dr. Hazen is a member of the active medical staff at Covenant HealthCare, St. Mary's Medical Center and Health Source Hospital in Saginaw.
